DLA is not my strong point, but aren't the forms the DWP send out stamped with the date that they are sent?

If it is, then you need to use this form as doing otherwise may affect when any change in payments would be in effect from.

I would give the DWP a call to find out which you should use, and whether there is an impact on you be not using the the original form.

It would be entirely up to you which form you use?

As the DLA Unit sent you the 'old' form, they are obviously prepared to accept it and make a decision on the information contained therein, but if you consider the newer DLA claim form (which I think you mean is a February 2011 version) would be more advantageous, you could complete that form if you so wish?

Either way, they are still both valid DLA claim forms for which a formal decision on entitlement is required.

Unlike a new DLA claim where the date stamp is crucial, renewal claims are not ordinarily dependant on the date of return. Indeed, the renewal claim may not even contain a 'return by' date?

The only penalty a claimant would suffer if he/she returned the form late, would be that there may be a break in payment until the renewal claim was decided, if in fact the decision was that DLA entitlement should continue? However, as you say, there would be no harm in seeking advice from the DLA Unit or the Benefit Enquiry Line to ascertain which form should be completed?

Sorry I am confused I received DLA1 Adult May 2010 as a renewal form would it be an advantage to use the new DLA1 Adult Feb 2011 form?

I really cannot face calling them.
