I wonder if anybody else has had this problem?

you could call me with regards with my DLA application (DLA1A) in regards to to question 60: Statement from someone who knows you.

I feel it is essential to my claim that my Community Support Worker answers this questions on my behalf as they are the person I have most contact from the Plymouth Teaching Primary Care Trust.

I have been inform that it is Plymouth Teaching Primary Care Trust policy not to assist by answering question 60 on form DLA1A.

Is this local or NHS policy?

I have in the past had a supporting statement on my DLA application by a CPN at my local mental health NHS trust.

However, if you can't get the statement you want, don't forget that the person giving the statement does not have to be a professional and that you can send in more than one statement, which means that if people know a bit about you, you can send in a collectin of statements in which each says what they know about you, so that it all adds up to giving the evidence you want to give.
