Just a quick tip that may have been said before I have added name, DOB and Ni number to top of each copy page and to extra sheets of information sent to both ATOS and DWP DM so if any separte sheets become 'separated or lost' they can be easily identified
I think what one of the mods said on an earlier reply is useful too, the page of the questionnaire you are giving extra info on, followed by number of question and then what it is in relation to. Got to make it as simple for the dwp don't we? Even if they make things really stressful and completely complicated for us!
