Hi Kayla,
In my experience it's never really been a case of the DWP or any case manager doing me wrong; just the DWP following bad advice from the assessor.
I would emphasise on the areas that will score you points and don't dwell on things which won't.
Keep it as concise as you can without missing any important information out and use your previous form as a template.
You can ask for a paper-based assessment which I did and luckily I was successful but I did have the back-up of my GP. If you can get a letter from your GP, practice nurse - or some other health care professional then include it. If not letters from friends - or relatives who know your struggles are better than nothing.
Certainly ask for a recording of the process if it does come to a face to face or a telephone assessment.

As you are still feeling very stressed about this form Kayla, try this.
Make two sets of diary pages, there are example entries and a template on here or just make a simple table with columns for date and comments. One set you fill in with all the problems of daily living and mobility as you have them. Remember how to count as " you can do" and therefore no points ONLY what you can do:-
Reliably, safely, as often as required, and in a reasonable [no more than 2x 'normal'] time, all as per the guides. So any thing that does not tick all 4 of those, put in with some explanation. Submit a copy of the diary, even a few days [a week would be great] as up-to-date evidence, they do seem to take a surprising amount of notice of these.
Use the second set for your private and personal encouragement logging things you managed which are triumphs, whether it is some work done on the form, managing to wash, or anything else. This balances the negative view in the one for DWP, credit to the Mod who thought of this originally.

I’m sorry for these questions and the last thing I want to do is waste peoples time. I also don’t want to sound thick in the process. It’s vital I get this form right as last time they made their own minds up going by my copy of their PA4 form from my assessment. I think I have now got it on how this form works. It’s basically to say since the last assessment they want to know any changes, but just to put the changes isn’t enough. You need to give the answer from the beginning from that date WITH the changes involved. I think I right in saying that. But if someone out there could confirm I would be extremely grateful. This is day 5 of the big brother form……small steps each time but now me and my partner get how to answer them then I hoping tomorrow is another day and we can get it done next week in time for the deadline. We should now by Tuesday or Wednesday next week and if I’m in any doubt I will ask my partner to ring and ask for that extension. To be honest I want it done, it’s been hell going to bed, lying there every night worrying, and waking up with it being the first thing on my mind…….it shouldn’t be like this but unfortunately it is, for me and most of the rest on this wonderful site.

We also have input from fellow members such as denby and phrank who have given you some very useful advice, as BIS has stated, take one question at a time, when you feel drained ... stop leave the form and chill out, make yourself a cup of tea.
It is of no use completing the form when you are stressed, that is when mistakes happen, follow our guides which the back office regularly up date, I know they are long but you only need to read the relevant parts to help you complete the form. Look at your form as if you were making a new claim, use extra pages if needed.
Any new condition you have, you must have had it for the previous 3 months and 9 months going forward.
Good luck and let us know how you get on, don't forget to send you form by recorded delivery and keep a copy.
