Pip review problem

I lost my last pip form, I still have some pages, problem is what should I write on the review one if they had scored me zero on certain activities even though I wrote explenation that I don't do them. My situation has not changed, so how do I fill in these activities they scored me zero for now? Do I just say I have no problems with them because they scored me zero. Or do I still wrote something.
