Hi ZenZen
If you need to send in any additional supporting documents send them to the DWP, the address of the DWP will be on any letter you have received from the DWP.
We cannot tell you which address to send it to as we do not know where you live, we also do not know who your assessors are, you can go to your local Welfare rights organisation, they will be able to help you with any contact details.

In terms of writing a complaint to the advisory service. is Do I have to put something about the
the Equality Act 2010, and do you know what discrimination should I mention?
