ESA new claim for work related ESA benefit

Hi All,
thank you for the advice.
so far so good.
I applied online and sent in the medical note from my GP stating i have an indefinite period of sickness. The Dr said this will last until retirement age for benefit/contributions purposes.
As stated above I have a complicated medical condition that is dementia related and has systemic impacts including bladder and bowel incontinence and inability to function safely.
I have received a backdated payment from DWP.
However, I have also received the capita WCA 50 form and had no notification of what group i have been placed in - is this normal?
Also the letter with the WCA 50 form states to send in further evidence such as letters and lists of medication, then further down the page says do not send in further evidence send them to the job centre instead!!
I have letters from other specialists stating it is dangerous to me and others to try and work - if I may ask, who should I send these too? I have no contacts at the Job Centre and do not know which one it would be optimal to contact.
As I cannot reliably use a pen (my writing has become un-readable even to me) I was going to ask my carer to fill in some parts and use dictation software to print off other parts. I did this with my successful PIP claim - will this approach be ok?
Also I will plan to use my PIP claim as a template as the answers seem to be similar and I only get a limited time in the day when I can function well enough to get things done - is this ok to do?
Finally - thank you for bearing with me - can the time be extended on sending my WCA 50 Form ( I had to get several extensions on my PIP application because of ill health)?

Thank u David. I called DWP and they have given me an address to post documents to. It’s the contribution based esa I have claimed for. I reach state pension age in 8 years I believe. But the prognosis for this condition is not good. It’s something like 80% systemic collapse/fatal within ten years of diagnosis. So trying to get things sorted family wise as soon as I can. Some folks last longer and die from cardiac issues instead and they are not really sure why. Either way I was diagnosed early and have made peace with things. Just striving to retain some dignity for me and the family moving forwards.

I have had no joy with getting a face to face with citizens advice (easier to jump over the moon)
I have had some calls with age concern who help with these conditions despite age and Im doing the WCA form now. Its heavy going - how people who can use a pen manage to fill it I dont know!!! Its impossible for somebody with bad motor skills
