I keep seeing this phrase used.

Can anyone tell me where I can find the "Beware tick boxes" guide please?

I can't think of a separate guide, but if you look at the guide relevant to the benefit you are claiming, it will take you through the form-filling process.

However, the thing to beware of with tick-boxes is that if you simply tick yes, no, or sometimes, you may not be giving them all the relevant information. By sometimes, you may mean almost all of the time, but someone reading it may interpret it as a little bit of the time.

Personally, I'm an annotater by inclination. If my answer doesn't fit the alternative answers, I say so.

It's also worth considering affixing extra pages of your own with any extra information you want to give the DWP about anything you are applying for. If they don't have the information, they can't take it into account. It's worked for me and for others.

Helen, just be as thorough as you can- i always put notes at the side of any tick boxes where I feel a tick may harm my claim. I also send a letter with all my details and a brief update of my protracted medical history to the DWP along with the forms and make sure the original form states that this letter of explanation is accompanying the form in the same envelope. Always send your forms recorded or registered delivery so as you have proof if they mislay it- as so often happens.
