FORM ESA 113 for the GP To Fill In.......

What questions is the GP and related health support, questions asked.
Have we any examples?.
Asking because ive come to the conclusion this more than anything else decides your fate with the ESA 50 when marked.
Ive decided to put together a printed sheet of how my mental disability effects me on a daily basis and Im going to hand one to my GP, psychiatrist and psychologist.
Im not so sure that 2 of the above will give a detailed account without any promting so Im going to set out self help for myself.
Any help here much appreciated.

Well well well, just look at question nos 6.
Does this not imply then that not being able to use public transport is an issue.?????? They obviously arent distinquishing between the two of taxi or public transport!!!!.
Well thats shocked me.
(wheres the exceptional circumstances applied here??)
Thank goodness ive seen this form.
Anybody else with any comments re- to this???.

I thought that it was down to the DWP to request GP's to fill out a 113?
Whether they actually bother to or not is another matter.
If someone prints off a 113 and takes it to their GP and asks them to fill it out, I doubt very much that they would.
The form itself is seriously lacking, as Derek says and may actually do more harm than good.
ie; asking if your patient has problems communicating with other people, yes or no?
they would likely tick 'no' if you can communicate with them.
Please can someone explain any advantages of a 113 to me?
