If your forum name is also your real name, you may wish to change it to maintain some anonymity.
This can be done quite easily by following the instructions on this FAQ :

WFI's can involve compiling CV's, giving information regarding previous employment, school and other qualifications, etc.

WFI's can be deferred at the discretion of the JCP Advisor, you can also request that the WFI is held over the phone, if you feel that your limitations preclude you from attending.
You cannot insist on the above though, and a supporting letter from your G.P. may help.
You would need to contact The JCP Adviser to discuss this.
My advice would simply be to fill the form out to the best of your ability, and not to worry.
It is not an exam !!
