4 years ago i had radiotherapy for prostrate cancer and got support group 3 years ago i got throat cancer and had my voice box removed am now total larygectomee. 10 weeks ago i hah a heart attack and spent 6 days in hospital, had a stent fitted. i got the form for work capibility yesterday, should i go straight to page 20 or because of heart attack fill all the form in antadvice please

This can be done quite easily by following the instructions on this FAQ :

I am really sorry to hear of your many serious health issues !
On Page 4 of The ESA50 it states :
"Are you having, waiting for or recovering from chemotherapy or radiotherapy treatment for cancer?"
If you tick "Yes", it states
"You don't have to fill in the rest of this form if you don't want to.
Sign page 18 and make sure that page 20 is filled in by a healthcare professional. This may include a GP, hospital doctor or clinical nurse who is aware of your condition."
Unless you "Are having, waiting for or recovering from chemotherapy or radiotherapy treatment for cancer?", you should fill the whole form in as it may be in your best interests to mention all of your medical conditions and any resulting limitations.
This is clarified by the statement on the same page :
"If you have other health problems, as well as cancer and the problems resulting from your cancer treatment, please fill in the rest of the form."
It is far better to include all information rather than take the chance of some of your limitations not being taken into account by The Assessors.
