I have my medical assessment on the 6th October. I wondered what to ask my Doctor to provide me with? Is it even worth it to take with me some sort of letter or report? My appointment with my Doctor is the day before my assessment so it may not be enough time for my Doctor to provide me with anything. Would it be possible to send it later?

I am extremely worried about this assessment .
Ideally you want your GP to write a letter that specifically supports the areas that you hope to score point for, if you are not seeing them until the day before you assessment then it is likely that it will not be available for you to pick up in time, if this is the case and even if it is available on time, you should send a copy to the DWP office dealing with your claim marked for the attention of the Decision Maker.
