I'm a little confused here.

Q2a What are your health conditions and Disability's, and approximately when each of these started ?

Is this particular question hubby's opportunity to explain HOW his multiple Disability's EFFECTS him,or. do you just make a list of what his Disability's are,and when they started ?

B&W members guidelines suggest :

"any PIP award will be based on the EFFECTS of your health conditions,so LIST them here .

No problem Noodles we are here to help.

The guide says any award....based on effects so list ALL of them here.

The point is to make sure you have listed all your hubby's health problems, diagnosed conditions, side effects of medication and possibly mental health problems like depression from being disabled etc.

You need to list all such hubby's illnesses, conditions etc and look at what evidence you have like consultant or specialist diagnoses etc. You may be able to get copies of letters from these people to your hubby's GP using your hubby's right to see his medical file and obtain copies of documents therein.

So I completely understand this,Would this be a good example ?

ALZHEIMER'S

After several years of what we first thought was forgetfulness,My husband was officially diagnosed with Alzheimer's in 2013. His illness effects both his ability to complete simple tasks such as remembering names,places,dates,familiar routes and even how to dress properly. This is all due to his short term memory loss which is part of Alzhiemers,a progressive disease of which there is no known cure. He cannot drive anymore cause his illness won't allow him to remember sign or information,he answers the phone but forgets who just called. He does have better days but because of his strong medication for his other issues listed below,he finds it difficult to stay awake and / or concentrate or Finnish up tasks now. He becomes moody and withdrawn,stares at people he does not know and finds mathematics very hard if not impossible and repeatedly asks the same questions. My husband forgets his own address,our phone number,his own history and finds planning and organising simple tasks distressful,fraught with errors and very unreliable.

For question 2a they are looking for a list of his conditions with approximate dates for when he started to be effected and when the condition was diagnosed.

You can provide more detailed information about how his conditions effect him, if not already covered in questions 3-14, in question 15 which covers Additional Information.
