No, they are not contractually obliged to provide letters of support and reports for their patients, even if the patient offers to pay.

No, there is nothing stopping you make a list of your limitations, care needs and mobility issues and asking your G.P. practice to put it on your records.

Yes, if you find that there are any errors or inaccuracies within your records, you can request that they are amended.

Hi C47,

PIP is awarded if the limitations and disabilities that you suffer as a result of your medical conditions indicate that you score enough points in the appropriate Descriptors of the applicable PIP Activities.

It is not awarded purely because you suffer with particular medical conditions, no matter what those conditions may be.

DLA to PIP transfers have not started in N Ireland, so if you live there, you will be reassessed under DLA criteria.

Hi C47,

If you can obtain letters of support from any treating HCP reiterating your medical conditions, and the limitations that you suffer as a result of these conditions, that would help.

If you can't, you can use the documents that you have, then point out the limitations that you suffer and tie them in to the applicable Descriptor for the relevant PIP Activities.

Both for ESA & PIP assessors.

I remember coming accross such forms, I am not sure the name of the forms,
but they were forms used by assessors and doctors for each category of disease.

there is also a list of all diseases for assessors and recommendations to assessors
for each medical contidiont. Where can I find this?

All the above is useful in case there is a need for appeal on the grounds that the assessor didn't follow rules, regulations and advice on those forms.

I gather that you mean "Descriptors" a list of these and all The PIP Activities that the Descriptors score points for can be found from the links that I have provided in : This Post.

From you queries, it is blatantly obvious that you have not read our PIP Claims Guide, I strongly advise that you do that first then pose any further queries that you may have.

Mods cannot possibly answer every query that pops into your head ! :)

I am not aware of a list of medical conditions regarding PIP.

You can download a copy of Guidance given to PIP Assessors from : This Link

Unless you have a fixed term DLA award, there is no way of knowing when your time will come to be invited to claim PIP.
