pip renewal form totally different from claim form was created by Dragonlady
I am hoping for a bit of help here my daughter has her pip renewal form to complete. I don't know if anyone realises but it's totally different to the pip claim form. There are no lists of tick boxes and points it's similar to one of the old dla forms. I had hoped that there would be a guide to filling it in but the only one on here is the actual claim form. Has anyone filled in the renewal form especially if you have a mental health problem ? Any help would be appreciated.

We haven't seen this form before, if you can email a copy to the office they will try and provide you some support for filling it in.

Failing that try and relate the questions on the new form to the PIP2, the criteria against which you will be assessed have not changed and so as an example; a question asking about your ability to prepare and cook a meal or anything similar needs to be answered in the same way as Question 3 on the PIP2, Preparing Food.
