What is a DS 1500 Form was created by redgt9
My wife sadly has been told she now has Lung Cancer and may be Bone Cancer so we was told to apply for PIP, I'm on ESA in a SG Income related and she is on my claim as a couple.
We are told to get a DS 1500 Form as this will help my wife to be able to make a PIP Clain, if she gets this would she still need a medical etc.

Hi rgt9,

I am sorry to hear such bad news !!! :(

You can only obtain a DS1500 Form for your wife If she has a terminal prognosis of 6 months or less, she can then claim PIP under "Special Rules" and her claim should be fast-tracked resulting in at least an Enhanced Daily Living Award (Possibly Enhanced Mobility also)

As this would be under Special Rules, she would not have to fill in a PIP 2 Form, nor should she need a face to face.

If she does have a 6 months or less prognosis, and DWP are rigid on this, you need to obtain a DS1500 from her G.P. , Consultant, MacMillan Nurse, etc which confirms this.

I would make some copies of The DS1500 to keep safe, and provide the original. You should inform PIP that her claim is under Special Rules from the very start of the claim.

You could also Contact MacMillan who do have Welfare Advisers who can help with "Benefits" claims as a result of cancer.

They should ensure that you are both receiving all the benefits that you are entitled to.

e.g If your wife does gain a PIP DL Award, this may entitle you to extra : ESA Premiums on top of your current ESA payments.

Even if her prognosis is not terminal for 6 months or less, she can still make a normal claim for PIP

Hi Bro, thank you very much for all the very useful information you given me, this will help me so much. I have spoken to our GP yesterday and he said he is going to issue this form on Monday. Again thank you.
