I have another ESA medical assessment in one week but DWP have not received my capability for work questionnaire which I sent 3-4 weeks ago. I had been feeling overwhelmed last couple of months and originally did not send form back in time but DWP said just come to assessment and you can talk it through which I agreed to.

I have fibro and m.e. so have bad pain and brain fog and memory problems and after thinking about it thought that talking it through would be worse for me, so I got some help and sent in the questionnaire. Luckily I took a copy of how I filled it in but very apprehensive of talking it through as my condition is so up and down and just so hard to explain. I am taking my husband with me so that will help I hope.

I was placed last time in the support group - thank you benefits and work! Why I didnt send the form in the first place was the whole thing is so overwhelming and I'm dreading going again. Have rung DWP quite a few times now and I HAVE to go to the new appointment as I had previously not turned up for two. I dont usually do things like that - but had sort of given up. I have resigned myself to going to the medical with a copy of the questionnaire plus my husband but need advice please. Thanks.

Your ESA50 would have been returned to the Assessment Provider Maximus, the DWP do not have access to this information and would be unable to confirm whether it had been returned or not.

So the first thing I would suggest that you do is to contact Maximus (aka HAAS and CHDA) to see whether they have it or not, if they do then you have nothing to worry about.

If they don't then there is still time to send a copy, although I would still take a copy of the form and evidence to the assessment,

The process is very intimidating, but at the risk of causing further worry I would give some thought as to how you will manage if you lose your ESA , this may give you the incentive that you need to deal with this re-assessment.

Best of luck

Gordon

Thank you Gordon. By the time I received the duplicate ESQ questionnaire form from DWP it was too late to send in before the medical tomorrow. I am taking a photocopy of original.

I will let you know how it goes.

Thanks again.
