Hi,
I an new to this site so please excuse the waffle. I have muscular atrophy, muscle wasting to legs & now arms too. Jut been through the pip thing, i had a lifetime award of high rate mobiity, middle rate care, had the home visit from capita, awaiting the result, now 3 weeks. Today i had the esa form, great start to the year. Last time I was put in the support group after a reconsideration, but I am worried about the result this time as all the questions seem to have changed.
First problem, who to put in the box about the person who knows my condition best, normally my gp, but he would already on the form !! I am basically house bound, cant go out on my own, rarely see he Gp, to be honest hate the idea of going out at all. I live with my brother & elderly father, who my brother spends most of the time caring for, dont want him running around after me too !!
Any thoughts would be most welcome

I would start by reading our two ESA Claim guides on the following link. The Physical guide covers questions 1-10 and the Mental Health one, questions 11-17

As to your question, do you see anybody at a hospital or clinic? You could mention their names, either in addition to or as a replacement for your GP. To be honest, in the majority of cases the DWP or assessor do not contact the persons on the form, although you should still name someone if you think that they will be supportive.

thanks for the reply. no i dont really see anyone else, stuck in the house & unable to travel on my own, brother has enough to do & is in poor health himself. i have read the guides, excellent by the way, & i will complete the form the best i can !! i am in the support group at the moment so will try & show that my condition is progressive & degenerative. I dont suppose i can put down the doctor/specialist who diagnosed me 12 years ago ?!!

This is an assessment of your limitations as they are now, if you haven't seen the specialist for 12 years then I think it unlikely that any report they would be considered relevant.
