Hi all
Sorry new to the site and I'm sure this has been covered but the search button is not being helpful today.

I have a small dilemma. I am smack bang in the middle of having to reapply for PIP and ESA and have got to the stage where I'm just about ready to get the forms filled out, starting with PIP as it has to be in by the 22nd, so, off I trott online this morning to try and find a downloadable digital version as my hands shake far too much to be legible, and I can't dictate to my carer as I can't speak. But I find to my dismay that there are only sample versions available. So, any ideas where I go from here or have I just missed the obvious? Is there somewhere where I can download a printable copy of the application form?

Hi Jo,

Welcome to Benefits and Work :)

I am afraid that there is no Digital PDF Version of The PIP 1 or PIP 2.

Mainly due to the fact that The PIP 2 Form that you receive through the post has a unique Barcode on it that directly ties the form to your individual PIP claim.

Therefore, only that PIP 2 can be used.

There is nothing to stop you referring to the PIP 2 Form, but typing your responses to the relevant questions on separate A4 sheets.

You should head each separate sheet with your Name, DOB, Date and NI Number and number each separate A4 1 of x, 2 of x, 3 of x, 4 of x, etc, etc, etc.

If you do this, you can simply write in the space provided for each particular question in the form, "See my response on separate added page number, 1 of x, 2 of x, 3 of x, etc, etc." (or words to that affect)

You can then attach the added A4 pages to the back of PIP 2 Form with your Barcode on it that arrived through the post, with Treasury Tags or something similar.

You should use our : PIP Claims Guide. (2nd Link down the page) whilst filling in the PIP 2 Form.
