If you are sure of this then you do not need to complete the form, but I would drop the DWP a note to say that you do not believe you are eligible.

I finally got my decision letter today after my ESA interview on the 8 April, thats over 9 weeks wait.
Anyhow, I have been put in the support group contribution based which is naturally a relief.
Is there any point in asking for my ESA 85 REPORT to see how I scored points for future reference, in case Ineed to refer to those points in the future, when I become due for re-assessment.
Also is it worth asking for my doctors report, which I understand they asked for, as maybe I could copy this to send in the next time my assessment is due, just in case my GP refuses to co-operate with them the next time round.
Can I ask when is my award next due for review, and would they know this at the DWP.
I think the maximum award is 3 years which would take me 2 years away to pension age, so they may not bother ?, wishful thinking.

Well done, terrify news.
You can request copies of the reports and find out when you are likely to be reassessed by phoning the DWP office that dealt with your claim, there should be no problem with any of your requests.
