To save her the stress of having to write out this detail all over again on the form I wondered if it would be ok to print out a copy of this statement and affix it to the form?

Yes you can attach a copy to the back of the form, but you/they really need to refer back to these problems for each of the ESA activities that they would expect to score points for, so it may be impossible to avoid repetition.
