Hi,
I have received a tribunal hearing date in a bout 6 weeks time so i needed eveidence to take to the tribunal so i went to my GP and asked for a data protection act 1998 so i filled the form in,first she the secretary said i can give you a summary that only included the medication and allergies thats it and she asked for £10 for that but i told her i wanted everything that my GP has on me so she said there are reports but i insisted and told her i want everything the GP has in hes computer so she said the reports aswell ,i said everything,so does the report include my hospital letters and spychiatrist information and etc...can you please explain to me someone,have i done the right thing and will the reports help me in my tribunal,will it be enough cos she said thats everything,i even told her i want letters which the hospital and my spychitrist has sent you she said we dont have that but i'll give you all your record ,is that correct but i dont know what she meant by reports i'll give you please reply and shes charging me £50 for that.

Hi Samkhan,

If your GP will not write a helpful report for you, obtaining sight of your medical records is the only way you can see if there is anything useful in them. You should receive copies of letters from specialists (even if they have not kept the originals, they should have been scanned into the computer if they were sent in the past few years) and of what your GP has written as well as reports of test results. Hopefully there will be something there that you can use to support your appeal.

ime also waiting for date toa arrive for my appeal ,,,i had to write a letter to my surgery to ask for medical records and copies ,i wrote in the letter what excactly what i needed ,, i had phonecall 2 days after saying theywere ready to be picked up, the charge was £25 in my surgery,,. was a full report suppose to have been but as i had changed my surgery 2 yrs prior alot of my hospital xtray results etc were not on my records ,i had to ring my old surgery who still had the information on the system and they kindy copied them out for me without any charge ,, ,ive completed my submission as to what and reason why ime appealing on the dscriptors ,have also had family members who care for me write statements also ,so maybe that would be a good thing for you to do if you can possibly,,as it will all help to build a picture for the tribunal as to your difficulties you have doing these activities day to day ,, my GP wasnt helpfull at all and wouldnt write me a detailed letter ,but ive managed to get all copies of mental health involvment and the letters they send to your GP about you ,so dont worry too much as if you do get these medical records it should be all in there ,, make sure you do copies to keep for yourself and send the other with all your infomation to the tribunal address ,make sure you put your ni number ,,ref no of your case and name etc on all your copies ,be uselfull if you could highlight certain things on your copy of medical reports so it be easier to look them up during your appeal if brought up in question so you can find it easier thats what i have done anyway ,,do wish you all the best at your hearing and hope you get the result your hoping for ,, :)

Hi thank you for the reply I got my medical records all of them which the GP had and was happy but i tried to read them they were a lot of blood test results and i tried to read some which were about my illness back dating to when i first got ill, i was trying to build a record of myself to this 2016 so they could see that i have along time illness record and some were old but to give them an idea what i suffer from,i managed to send to the tribunal about 15 pages but they were roughly about 100 pages of medical record the GP gave me and also i made copies of the 15 pages and put my name and date of birth and pip reference number so they dont get lost and also i got a phone call today am going to see my citizen advice bureau this Tuesday and they said to bring to bring the documents concerning the pip and DWP has sent me,the application form i filled in for the claim and all that also to them i'll be taking the 15 pages copy i made and sent it to the pip but shall i take all the rest of my medical records which i kept to the side and were not in handy just in case if they could find something valuable in it but its a lot of papers(record) and they said that it will take about 2 weeks for them to read and my appeal is on 22nd of September and if they do find out something good then will i have time to send the rest if the CAB find something to send because you have to send the document to pip before 7 days of your appeal date because it will reach them in 4 or 5 days and the 7 days notice so that will make it 12 days am really worried am confused and have i done the right thing and is it too late for me to send any more documents to pip if the cab tells me too or can i make it in time,please give me all the right information to what i have said whether i done the right thing or not cos everything is creeping on me ASAP.THANK YOU R.ZAMIR

I'm sorry, I am not to sure exactly what your question is! If you are asking whether you should take all your documents with you to CAB, then the answer is 'yes'.

hi,.
yes ide take all your medical copies to CAB when you go as they can double check if there anything relevant there you can use ,,, , if there is any evidence you have as long as it sent to the tribunal before 7 days of the date it should be fine i believe, but ide send them in recorded delivery and first class to be safer they get there in time ,, you dont send anything off to the dwp at this stage , any evidence you should send to the tribunal address ,,and make sure you have copies for yourself ,, and put your ref no; and NI number etc on each paper ,,, everything you send to the tribunal they send you copies of it back all numberd accordingly ,,
