I have been receiving pip for nearly 4 years, after having more tests etc. I have deteriorated. Should this be sent off to PIp? I not too sure how everything works.

Whilst you must report any improvement in your conditions, the DWP are on the whole a lot less bothered if claimants don't report a deterioration.

The question to ask yourself is whether the deterioration is sufficient for you to receive a higher award, if you do and I would encourage you to be pessimistic in looking at the extra points you might get, then you should consider reporting a change of circumstances, if the situation is not clear then you might want to be more reluctant to contact the DWP.
