SAR DWP address

I am even thinking that she contacted the DWP to request them to send me a PIP form, I was on DLA, and requested to be sent a copy of my PIP form along with all information I sent to PIP, on 13 June 16.

I have a feeling she has contacted the DWP to request all the info I sent for my PIP.
She also requested I meet my new GP about ten times during the home assessment for home care package. And suggested she comes to my home on the day I have my GP appointment, to see how I manage...how I go down a few steps, how I use my wheelchair, how I am at the GP, and return home with me to see how I manage the steps, (with the help of my carer). I will not do it, based on her previous assessment full of lies.

Distressing though this is for you, DWP would have committed a huge breech of confidentiality if they had given any information about you to anyone who requested it!

on the SAR form theres no box for ticking in anything for PIP ?
Just esa dla etc?
how do I go about so they know its to do with pip
