Telephone interview details

In your members guide on page 11 it says "Once you have completed the telephone interview you will be sent a copy of the completed form to check, sign and return"
I have received a 40 page form the content of which seems to correlate with your guidance on completion, however the form is totally blank and does not include any of the information provided by me in the initial telephone interview which I assume from the notes referred to above it should ?
Or should I have received a further form with details of the information provided over the phone ?

The initial telephone 'interview' was just to collect your basic personal information - Name, address, telephone number, National Insurance number, details of any hospital stays etc, any time absent from this country and so on.It's a way of deciding what category of health issue you have - physical, mental health, whether you have a learning disability which means you will need help to fill in the form etc.
The form they have now sent you is the one you fill in, with the help of the guides, and which includes details of all the medical professionals you have seen, (GP, any Consultants etc) and explains fully how your illness affects you and how it prevents you from carrying out various activities. On page 12 of the guide you will find a heading 'How your disability affects you) and here is where you will get the information you need to help you to fill in the form.
If you work your way carefully through the whole guide you should be able to pick out which descriptors actually refer to you and your disability, and will see how to explain to the decision maker/assessor via the form the actual difficulties you have.
I hope this explains where you go next; it is confusing, but you have done the first bit on the phone; now you have to do the hard bit and use the guides to help you through it.I hope it gives you all the help you feel you need. Good luck with the form.

I may not have been totally clear in my initial post, when I referred to "Once you have completed the telephone interview you will be sent a copy of the completed form to check, sign and return" I took that from the PIP guidance on the members pages.
I am a little thrown as it refers to a form to "sign, check and return". I have received no such form, just the 40 page one to complete and return with evidence to support my claim (I have been receiving Higher rate DLA and Mobility for 29 years due to MS and like many more now need to apply for PIP).

When PIP was first introduced the DWP were sending out the PIP1 for signature, now it rarely happens.

Sorry to jump into the topic
I am due to move from DLA to PIP, i do not answer the phone to unfamiliar numbers
and use Caller Display.
Basically how will PIP 1 be initiated if i do not participate in the in the transitional phone call?
