Completing PDF ESA50 was created by digitalrace

Hello
Completing PDF ESA50 in anticipation of reassessment at the end of August. The PDF has 'ESA50 04/15' in the bottom right hand corner of the form.
Firstly,is this the current ESA50?
Secondly, when I type in the boxes the boxes disappear and then return to pink colour. When I print the form there is the my type but no boxes. Is this normal and is it acceptable to DWP?
