ESA renewal

ESA renewal was created by mistake
Hi everyone, I was transfered from Incapasity benefit to the support group of ESA three years ago. Three years on I have therefor been waiting for the brown envelope to arrive for renewal.

A form Esa50 04/15 arrived on Saturday but was in a white envelope and came from thr Health Assessment Advisory Service. I am just wondering if this is the norm now as previously the forms went via job centre plus but this time is to be returned to Centre for Health and Disability Assessments in Leeds
Sorry if something similar has been posted recently, it has been a while since I signed in on the forum.

Your last ESA50 would have been sent to you by ATOS, they were replaced last year by a company called Maximus who operate under the name of the Centre for Health and Disability in regard to ESA assessments.

You should use the latest ESA Claim guides to help you fill out the ESA50
