eyesight problems and filling out forms

due to severe sight impairment it is not possible for my wife to fill out the PIP form, or sign it. if i fill it out for her, do i then sign it in my name and print my name in the box?

If you are going to act as secretary for your wife and record her words, then use the first person 'I cannot do etc' and sign the form on her behalf.

If your wife has little insight into her condition then use your own words in the third person 'she cannot etc'.

Either way, explain what you have done and why.
