I've received a letter calling me to a medical in just under 3 weeks (After a 22 week wait!). I've also just received a report from a psychologist confirming a new diagnosis of an eating disorder, which also states I've been referred for a course of therapy + references to my severe anxiety, depression and physical problems.
The letter from Medical Services requests patients bring reports, along with medications, etc. But I wonder if it might be better to submit the information to the DWP before the medical?

Send it to the Assessment Provider rather than the DWP.

Al;so if you have kept a copy of of your PIP2/ ESA50 take that with you
,At a recent WCA I attended an important page (5) of ESA50(it lists the medical conditions a claimant as listed ) was not with the HCP's copy.

Should I send the original report or a copy to the assessment provider?
I also intend to send a brief cover note with my NI number and brief points as to the content. Should I bother with that or just write 'further evidence'?

Never use the original document unless specifically required to do so.
Anything that makes it easier for those assessing you to find to information is always a good idea.
