PIP assesment was created by john kelly
Had my pip assessment today, i brought extra documents to support my medical condition the health assessor requested i sign a form so she could photocopy these document (sign and print name) next to the x she had placed on the form. I did not have time to read the form which had several pages attached as she continued to ask questions.She did return the documents later at the waiting room but on checking when i got home i did not get a copy of this form. Is this normal practice should i request copy of form from atos.

the documents were originals but told the assessor she could keep them as i had copies at home. the information was letter from my heart specialist updated medication future tests and clinic appointments

It may be because they are originals, I'm afraid I don't know what the document you signed was for, you could try contacting the Assessment Provider for a copy.

This happened to me also, my eyes are not good so I was unable to see what the paper work I signed said. What she told me was, I needed to sign because this was additional information and felt it was worth sending in.
