In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ
My full name is showing, how can I stop it?
When you make the initial phone call a shorter form will be completed for you. Then the PIP2 form will be posted out and you will have 4 weeks in which to complete it - this includes postage both ways. A 2 week extension is usually given on request - just phone DWP once your form has arrived if you think you will need it.
Our PIP guides talks you through the process, including the information you will need to hand when you make the initial call.

May I suggest that you bookmark/favourite this on your web browser now so that you can find it easily in future? This will allow you to return with further questions or comments about your PIP along the way without having to start a new topic each time. It helps us enormously when everything relating to the same claim is kept in one topic - and I hope you will find it helpful too
