Keeping diary for PIP application

Even this small task causes me great pain and I suffer that day and the day after, but it gives me some sense of purpose and self worth. I'm frightened of mentioning it, even though I know someone could technically be on PIP and be at work or college. OK I may seem paranoid but how do the minds of assessor work? I think that's a rhetorical question.
