My full name is showing, how can I stop it?
You can contact DWP and ask for a copy of your ESA50 form - but I wouldn't expect them to release it until your claim has been decided.
It is Maximus, not ESA, who arrange the face2face assessments - I won't call it a 'medical' as it will be unlike any other medical you might have undergone! Our ESA guide explains what to expect on the day.
ESA claims and appeals
The claimant is allowed to change the appointment only once without penalty so make sure your partner has his diary to hand when you phone up to do this.

I have heard nothing from DWP yet. My question is should I phone them or send in my g.p's letter or both.
