Yes you can supply new evidence at the assessment, ask the assessor to read it and include it in your file before they start the interview, they are required to do this by the DWP.
If you are only able to manage the assessment while in bed then that's fine, I assume that you will have someone with you to let the assessor in.

Yes my Husband will be with me because he only works a few hours a week and my daughter comes and visits and helps when he is away due to the fact of the falls and accident I have

PIP Decision are taking about 204 weeks at the moment.
You should be able to request a copy of your assessment report by contacting the DWP office dealing with your claim.
If you have a definitive statement about your memory problems before the Decision is made then you should send a copy to the DWP.
