Completing pip form

The questions being asked are for different things. So as some examples; using an aid does not mean that you need assistance and needing prompting does not mean that you need to use and aid. Of course you may need both but for different reasons, so you might need prompting to prepare and cook a meal due to depression and an aid to help you complete it due to a physical impairment.
You need to answer each question as relevant your limitations, either by using the tick boxes or in the comments section below.
For question 6 you are saying that you can only wash with the use of the bath seat, Any rituals that you have with your washing would be something you should detail in comments box or additional sheets.

OK let's try this again!
The questions are independent of each other and all of them must be answered.
You need to read and answer the question that is being asked, at the moment you appear to be second guessing your answer to one based on your answer to another one.
Let's take Preparing Food. The numbering is mine to signify how you should approach the questions.
1. If you can cook a meal without problem then answer "no" to (a) and (b) and say you can prepare and cook a meal without problem in the comments box.
2. If you can't cook a meal without some form of help then or you can't prepare cook a meal at all then;
3. Can you cook a meal with the use of suitable aids?
If you can't cook a meal, even with the use of aids then you should answer "no".
If can cook a meal when using an aid then answer "yes"
If sometimes you can and sometimes you can't then answer "sometimes"
In all cases explain the comments box why you have answered as you have and in the case of "sometimes" how often you are unable to.
4. Can you cook a meal with the help of someone else. It does not matter what you answered to (a)
If you can't prepare and cook a meal even with prompting, assistance or supervision then answer "no"
If you can prepare a meal if you receive one or more of prompting, assistance or supervision then answer "yes"
If you sometimes need prompting, assistance or supervision and sometimes don't then answer "sometimes"
In all cases explain the comments box why you have answered as you have and in the case of "sometimes" how often you are unable to.
The approach is the same for the other activities.
So for Washing and Bathing you have said that you need to use a seat (yes to aid) and that you need have assistance to wash (yes to someone else helping). You then need to explain why you need the seat and why you need someone to wash you.
