I'm completing my ESA50 (nightmare) can I request a home visit for the face to face assessment, if Im called for one, which I probably will be, which is already causing me major anxiey ! on the grounds of depression/anxiety/Agoraphobia? I see no place on the form for this?

There's a section for Other Information at the end of the form. You will need a letter from your GP explaining why you cannot attend at their offices to have any realistic hope of a home assessment.
