I was put in the support group 2 years ago due to physical problems, no mental. I do get premuim payment. Im still in support group now. Its coming up to 2 years for re assesment. Nothing has changed in my condition. Will i have to go again? Will they send me me forms and what do i write in them. Im worried.

Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is
Welcome to Benefits and Work
All ESA claimants should expect to be reassessed on a regular basis, the maximum period for the Support Group is usually three years.
If you contact the DWP office dealing with your claim then they can tell you when this is likely to take place.
When you are assessed you should treat it as a new claim and complete the new ESA50 in detail and supply any relevant evidence that you have even if it was submitted before.
Our ESA Claim guides can be used to help you complete the form.
