Historic Supporting Documentary Evidence

I keep an ongoing 'live' PIP file that has daily diaries, consultant and GP appointments.

It will be a substantial document when it comes to complete whichever form I am required to.

I will also make sure that there are recent consultant reports confirming my current prognosis regarding each diagnoses.
