ESA50 form pg 5 suggeation - diary of symptoms was created by sue77
Hello
I'd be grateful for guidance please?
I'm completing a reassessment for ESA ( in support group since 2013) and need to submit it this week. On page 5, they suggest other information you might like to include - a diary of your symptoms...if your health condition varies from day to day. This is something I used to do though not for the last 6 months as not be able to do this. I'm wondering if it would help or not to submit something? I f by doing that I would then show I'm more capable of doing something than I actually am at the moment, I'm unsure? For each question I've put examples for each of my answers as to how each condition varies anyway.
I've written it all in draft form and will then copy it onto the actual form. It's taken much longer than I remember in 2013 and hope I get it done in time......I know that's important.
Thank you
Sue

Nothing on this board constitutes legal advice - always consult a professional about specific problems

Assumptions will definitely be made if you complete the diary, so you need to balance the benefits of explaining how your problems change from day to day against those assumptions. Keeping a diary will typically show reasonable memory and an ability to complete tasks.
