My partner has been getting Dla high rate mobility middle rate care since having 2 heart attacks in 2002 he has got a letter telling him to apply for pip don't know if the same rules apply in Northern Ireland but would you know how he would go about asking for home visit or do you think he would be better travelling to see Hcp if requested he is really out of breath due to heart condition would like some advice re filling in form x

First of all have a look at our PIP Claim guide which covers, amongst other things, competing the PIP2 claim form.

To have any chance of a home assessment you will need your partner's GP to write to the Assessment Provider with an explanation of why they cannot attend at the AP's offices.

As far as I know Ann, in Northern Ireland the assessments are done by Capita and in your own home
