Hi Gordon Have downloaded the pip GO from and given it to my doctors to complete for me so I can submit this with a covering letter at my home visit. My worry is , will using the form that is obtained from your site, prejudice Capita against me as they will see I have been seeking your guidance ?? Please advise as best course of action
