I'm afraid you do, you need to treat this as a new application for ESA and complete the form in detail and provide evidence to support your claim as you can, even if you have sent the information before.
Please use the latest ESA Claim guide to help you complete the form, although there have not been any changes to the Descriptors since 2013 the DWP guidance has been updated.
