The PIP Appeal guide has links to the SSCS1 form that you will need to complete and the Submissions guide has a complete example that you can use to help you fill it in.

I assume you cannot print the form, can a friend or neighbour? Some Post Offices, libraries and stationers have print facilities that are relatively cheap.

You can try contacting the Tribunal Service and your local JC+ office might have a copy.
