I recommend that you get face to face advice before you make a decision to do this.
As a couple you can only have one Income Related claim so one of you will have to close their ESA(IR) claim.
Also if you are receiving the Severe Disability Premium (money because you are severely disabled), then you will lose this because you will no longer be living alone (unless your partner is also receiving a qualifying benefit).

Do an internet search for "welfare advice" with your postcode, town or county.
Your partner needs to be receiving
- Attendance Allowance (or Constant Attendance Allowance paid with Industrial Injuries Disablement Benefit or War Pension)
- Disability Living Allowance care component at the middle or highest rate
- Personal Independence Payment daily living component
- Armed Forces Independence Payment

Yes I your partner is receiving DLA MRC then this should enable your SDP to continue in payments, in fact you should be entitled to the couples rate which is about £120 per week.
There is no need for your to contact the DWP unless and until you move in together.
There are a variety of agencies still providing welfare advice but it will depend on where you are.
