Hi. I am at the point of movingfrom dla to pip. I recieved a txt.from dwp saying my pip form would be sent out shortly. How can I access my dla reports as this may help fill out the pip form?
Also,how do I appointent someone to help me please.?
I don't think I can face these people alone...
Also,how would I be able to get permission for recording any iterviews,as I gather some are prone to lying.

The surest way is for you to issue a Subject Access Request for the documents, although I can't guarantee that this will be dealt with by the time that you need to return the PIP2. You can try calling the DWP office dealing with you claim, but if your last DLA assessment was some time ago, then it is likely to be in offsite storage and will need the SAR.

To record the assessment, you will need to request permission from the Assessment Provider, I recommend that you write to them with the request. You will need to be able to record simultaneously directly to tape or CD, you cannot use a digital device.

Who would be my assesment provider?

Whick link do i click on pls as i have the choice of two. Would it be worth my while chasing up my records do you think,- to help with my pip form?Does it cost anything?

Also,what to do about requesting an appointee to help?

Which Assessment Providers it is will depend on your postcode. if you are required to attend a face to face then they will contact you and you can make the request then.

One link is for the form you will need to complete and the other is instructions on what to do with it. There is no fee.

Do you mean an appointee or an advisor? An appointee has a special meaning for the DWP and would be someone who takes control of your benefits because you have been deemed as unable to manage them yourself.

If you mean an advisor then try doing an internet search for "welfare advice" with your postcode, town or county. You can also try any charities that specialise in your conditions.
