Hello. Do you do a template letter for requesting all information held on file for DLA?
They ask if you would like them to consider any evidence held with the current DLA claim, when doing the initial phone call. for pip To let members know, you need to in writing, ask for all records and you need to do this either before you make the initial phone call, or once you have done the call, you have to speak to someone in the PIP dept, not DLA dept, as they told me today. Was confusing and a bit like a game of telephone table tennis! Do you have a template letter please.

You need to make a Subject Access Request.
This can either be a letter headed "Subject Access Request"
or the DWP have a form that you can use.

i have just bumped up against this issue - i did an online request for my original DLA application and all asscociated paperwork - received a confirmation, by mail that it had been received and i would get a response in 4 wks - 6 weeks later i tried to chase it up
i was informed that my records had been transfered to the PIP department at the time when the PIP to DLA transfer letter was sent to me - but my request has not been passed on to PIP and i was not informed that PIP had my records - and i thought the Department of Stealth and Total Obscurity was dead!
