Hi All Hope everyone is well. My consultant is more than willing to write a report/letter for me to include in questionnaire. Could anyone suggest just what he should include ? I have spinal problems - had large fusion 21/2 yrs & six months later site opened again 21/2 yrs & many procedures later now told have to have 2 further serious ops & hip replacement. Lots & lots of mobility issues which have got much worse plus sever pain. I would be very grateful for a info X
