Helping a friend with the form was created by kah22
In previous posts I’ve mentioned that I’m helping my friend with her PIP form. She suffers from Bipolar 1. In essence we are talking the questions over but I’m actually doing the writing and forming the sentences and paragraphs for her because she’s on a bipolar low at the moment and is not fit to fill in the form herself
The problem I’m having is in how to phrase an answer e.g in Q 12a I could write
Because of my bipolar I have problems in motivating myself in preparing food and can be a danger to myself as I have injured myself in the past
OR ALTERNATIVELY
Because of Name she has problems in motivating herself to prepare food and she can be a risk to herself as she has injured herself in the past
Basically the same wording except it has changed from the First to the Second person
We have prepared detailed answers to each question. Most answers easily cover 1/2 typed foolscap. Not something that a person who is currently fighting bipolar depression could easily do and certainly something an assessor could mark you down upon. So how do I actually introduce the reply? Is there a phrase I use

I should say that she won’t see a CAB official or someone like that because she thinks they are busy bodies

As you are preparing the document then the key issue is whether you are recording the claimants words or you have decided on the content, from your post it is the former and you should use the first person.
Eplain in the form, how it has been produced, emphasising that all of the typing, formating, spell checking, etc. is your work, based on the claimants descriptions of their limitations.
