Hi there, I’m a new member having received my first phone call. I’ve now had the form to complete where it asks if you have any health reports to add. My osteopath is more than happy to write something for me but neither of us are sure what to include. Any help/suggestions much appreciated. Thanks

I assume that this is for PIP, there is a GP Notes document in the PIP area, you can either give it to your Osteopath to fill in or use it as a template for them to write a letter.

Thanks Gordon, yes it is for changing from DLA to PIP and good idea re using the same template.
