Hello. I have been on DLA for many years, and was granted it for good in 2004. But now I just received the PIP application form, and seem to be stumbling at the 1st question (Q2)(health condition & disability) I was diagnosed with ulcerative colitis & Crohns disease many years ago, and This was followed by 8 major operations & 40+ smaller surgeries, all of which have contributed to my varies health conditions that have come from this. Should I add all these in the list? I am so worried that i am not answering this correctly. I would be most grateful for your advice. Shan Annette
